Socioeconomic status (SES) is an economic and sociological combined total measure of a person's work experience and of an individual's or family's economic access to resources and social position in relation to others. When analyzing a family's SES, the household income, earners' education, and occupation are examined, as well as combined income, whereas for an individual's SES only their own attributes are assessed. Recently, research has revealed a lesser recognized attribute of SES as perceived financial stress, as it defines the "balance between income and necessary expenses". Perceived financial stress can be tested by deciphering whether a person at the end of each month has more than enough, just enough, or not enough money or resources. Biologic variability and dramatic changes of brain development in children aged 0 to 2 years make it challenging to accurately detect subtle abnormalities in single Magnetic Resonance Imaging (MRI) scans. Diffusion MRI (dMRI) indices such as Apparent Diffusion Coefficient (ADC) are reliable measures of water content in the brain and thus an excellent surrogate marker for brain development. Developing robust age-specific diffusion biomarkers for quantitative measurement of normative brain evolution would enhance our ability to detect subtle alterations due to tissue injuries or neuropathological disorders. Obtaining significant numbers of normative MRI scans for this age group means redirecting clinical data from hospital databases for research purposes. Therefore, this pilot project demonstrates the feasibility of identifying, retrieving and analyzing pediatric clinical dMRI data to investigate normal brain development from birth to 2 years. Research Patient Data Registry (RPDR) at Massachusetts General Hospital (MGH) was used to collect patient medical information and identify healthy children according to radiology reports. Corresponding MRI data were retrieved from MGH Picture Archiving and Communication System (PACS) using the prototype of Medical Imaging Informatics Bench to Bedside (mi2b2) software. A specific pipeline was created to handle the volume of studies and extract technical scan information used to identify comparable diffusion series; 193 studies were used for analysis. Two markers, whole brain average of ADC and Fractional Anisotropy (FA) values (WBAADC and WBAFA), were computed for each patient, their age-evolution across patients was investigated with different models. WBAADC and WBAFA seem to exhibit biexponential decay and increase respectively and might be gender-specific. These results have clinical implications for potentially determining the health status of an unknown individual, and research utility for continued development of these tools.

Longitudinal study of neonatal brain tissue volumes in preterm infants and their ability to predict neurodevelopmental outcome

Premature birth has been associated with poor neurodevelopmental outcomes. However, the relation between such outcomes and brain growth in the neonatal period has not yet been fully elucidated. This study investigates longitudinal brain development between birth and term-equivalent age (TEA) by quantitative imaging in a cohort of premature infants born between 26 and 36 weeks gestational age (GA), to provide insight into the relation of brain growth with later neurodevelopmental outcomes. Longitudinal T2-weighted magnetic resonance images (MRI) of 84 prematurely born infants acquired shortly after birth and TEA were automatically segmented into cortical gray matter (CGM), unmyelinated white matter (UWM), subcortical gray matter (SGM), cerebellum (CB) and cerebrospinal fluid (CSF). General linear models and correlation analysis were used to study the relation between brain volumes and their growth, and perinatal variables. To investigate the ability of the brain volumes to predict children's neurodevelopmental outcome at 18-24 months and at 5 years of age, a linear discriminant analysis classifier was tested and several general linear models were fitted and compared by statistical tests. From birth to TEA, relative volumes of CGM, CB and CSF with respect to total intracranial volume increased, while relative volumes of UWM and SGM decreased. The fastest growing tissues between birth and TEA were found to be the CB and the CGM. Lower GA at birth was associated with lower growth rates of CGM, CB and total tissue. Among perinatal factors, persistent ductus arteriosus was associated with lower SGM, CB and IC growth rates, while sepsis was associated with lower CSF and intracranial volume growth rates. Model comparisons showed that brain tissue volumes at birth and at TEA contributed to the prediction of motor outcomes at 18-24 months, while volumes at TEA and volume growth rates contributed to the prediction of cognitive scores at 5 years of age. The family socio-economic status (SES) was not correlated with brain volumes at birth or at TEA, but was strongly associated with the cognitive outcomes at 18-24 months and 5 years of age. This study provides information about brain growth between birth and TEA in premature children with no focal brain lesions, and investigates their association with subsequent neurodevelopmental outcome. Parental SES was found to be a major determinant of neurodevelopmental outcome, unrelated to brain growth. However, further research is necessary in order to fully explain the variability of neurodevelopmental outcomes in this population.

Calnexin Controls the STAT3-Mediated Transcriptional Response to EGF

Calnexin is a well-characterized transnnembrane chaperone involved in the folding of newly synthesized glycoproteins in the lumen of the endoplasmic reticulum (ER). Here, we reveal a previously unrecognized function of calnexin in regulating the transcriptional response downstream of epidermal growth factor receptor (EGF), the product of a well-known human oncogene. We find that cell stimulation with EGF leads to the caspase-8-dependent cleavage of the calnexin cytoplasmic domain, preferentially at ER-mitochondria interaction sites. The released fragment translocates into the nucleus, binds to PIAS3-a natural inhibitor of activated STAT3-and, thus, acts as an enhancer of the STAT3-mediated transcriptional response to EGF. Also, we reveal the unsuspected capacity of calnexin to sense ER stress and, in response, prevent the EGF-induced processing of its cytosolic domain. Thus, cells integrate the health status of the ER to determine the amplitude of their response to EGF.
