A microfluidic cell-trapping device for single-cell tracking of host-microbe interactions

The impact of cellular individuality on host-microbe interactions is increasingly appreciated but studying the temporal dynamics of single-cell behavior in this context remains technically challenging. Here we present a microfluidic platform, InfectChip, to trap motile infected cells for high-resolution time-lapse microscopy. This approach allows the direct visualization of all stages of infection, from bacterial uptake to death of the bacterium or host cell, over extended periods of time. We demonstrate the utility of this approach by co-culturing an established host-cell model, Dictyostelium discoideum, with the extracellular pathogen Klebsiella pneumoniae or the intracellular pathogen Mycobacterium marinum. We show that the outcome of such infections is surprisingly heterogeneous, ranging from abortive infection to death of the bacterium or host cell. InfectChip thus provides a simple method to dissect the time-course of host-microbe interactions at the single-cell level, yielding new insights that could not be gleaned from conventional population-based measurements.

Quantitative Single-Cell Analysis of Host-Pathogen Interactions

During infection, microbial pathogens encounter phagocytic cells of the host innate immune system, such as macrophages and neutrophils. These encounters typically lead to uptake and killing of the bacteria by the host cell or, conversely, parasitization of the host cell by the bacteria. Microscopic analysis of these host-microbe interactions is complicated by the fact that phagocytic cells are motile and tend to stray out of the field of view. Most host-pathogen studies have therefore been undertaken at the population level. However, the success of many pathogens likely lies in their ability to generate heterogeneity within the population, facilitating adaptation to every host micro-environment and to environmental perturbations (such as application of an antibiotic). We believe that studying host-pathogen interactions at the single-cell level can reveal not only new mechanisms of pathogenesis but also why current treatments are not always successful. This thesis presents a novel microfluidic device for the long-term co-culturing of phagocytic cells and bacteria within spatially confining microchambers. Each device is patterned with thousands of microchambers, permitting high-throughput fluorescence timelapse microscopy of host-microbe interactions at single-cell resolution. We demonstrate the utility of this approach by co-culturing an established host-cell model, Dictyostelium discoideum, with the extracellular pathogen Klebsiella pneumoniae. We observed that K. pneumoniae is readily taken up and killed by D. discoideum but some events can take ten times as long as others. The capsule, a polysaccharide layer present at the surface at the bacterial cell wall, reduces the phagocytosis efficiency by 1.5 fold, but does not influence the killing time. The opportunistic human pathogen Mycobacterium marinum was then used to study intracellular bacterial virulence. As expected, bacteria could infect host cells, replicate inside, and eventually lyse them. However, surprisingly, in the majority of the infections, the bacteria were actually released from the host, with no apparent harm to either the host or the bacteria. Less frequently, bacteria were even seen being killed. The timing of these events was highly heterogeneous, ranging from a couple of hours to several days. In summary, the tools presented in this thesis provide a new and simple approach for following individual host-microbe interactions at high spatiotemporal resolution from the onset to the end of infections. This work unveiled that the timing and outcome of host-microbe interactions are unexpectedly heterogeneous, which may have important consequences for the bacterial pathogenesis and ultimately for the development of future treatment of microbial infections.

A microfluidic cell-trapping device to study dynamic host-microbe interactions at the single-cell level

Single-cell imaging of host-microbe interactions over time is impeded by cellular motility because the cells under scrutiny tend to migrate out of the imaging field. To overcome this technical challenge, we developed a microfluidic platform for imaging hundreds of individual motile phagocytic cells and bacteria within microfluidic traps that restrict their movement. The interaction of trapped host cells and bacteria is monitored by long-term time-lapse microscopy, allowing direct visualization of all stages of infection at the single-cell level. The medium flowing through the microfluidic device can be changed quickly and precisely, permitting the real-time imaging of cellular responses to antibiotics or other environmental stresses. Here, we demonstrate the potential applications of this approach by co-culturing the phagocytic amoeba Dictyostelium discoideum with the intracellular pathogen Mycobacterium marinum. However, the platform can be adapted easily for use with other host cells or microorganisms. This approach will provide new insights into host-pathogen interactions that cannot be studied using conventional population-based assays.

Microtissue models for studying the dynamics of host-pathogen interactions in the urinary bladder

Urinary tract infections (UTIs) are amongst the most common bacterial infections and are the second-most frequent reason for antibiotic prescriptions. Moreover, in about 25% of all treated cases, recurrence of infection occurs. Uropathogenic Escherichia coli (UPEC) is the most common causative agent of UTIs. Much of our current understanding about UTIs has come from mouse models of UTI which have highlighted the intracellular lifestyle of this pathogen in the forms of intracellular bacterial communities (IBCs) as the prominent cause for bacterial persistence. Recurrent infections are thought to be caused by bacterial fluxing from persistent IBCs in the umbrella cell layer of the epithelia in the bladder. Although in vitro model systems of UTIs have been developed to study acute phases of infection including the formation of IBC and their dispersal, they suffer from certain shortcomings. For example, these models lack vasculature and the possibility to impose mechanical stresses, such as those experienced by the cells during bladder filling and voiding. Moreover, the design of these in vitro systems makes it difficult to study long-term UPEC persistence when exposed to antibiotic or neutrophil-mediated stresses. In the last two decades, two bioengineering approaches have emerged to generate functional and physiological tissues: organoid and organ-chip systems. Differentiating cells self-organize in a hydrogel and enable the formation of organoids, which mimic various aspects of organ physiology and function in a three-dimensional structure. Organ-chip systems rely upon co-culturing pre-differentiated cells across a synthetic membrane in a defined geometry. The organ-chip systems allow dynamic control over environmental (nutritive) state and over the mechanical stresses experienced by the emulated tissue. In this thesis, I showcase the development of model systems to study UPEC pathogenesis using both these approaches. In the first part of this thesis, we developed a bladder organoid model of UPEC infection that recapitulates the stratified bladder architecture within a volume suitable for high-resolution live-cell imaging. Bacteria injected into the organoid lumen rapidly enter umbrella cells and proliferate to form IBCs. Unexpectedly, we identified populations of individual âsolitaryâ bacteria that form independently of IBCs and which penetrate deeper layers of the organoid wall, and evade killing by antibiotics and neutrophils. Volumetric electron microscopy revealed that these solitary bacteria may be intracellular or pericellular (sandwiched between uroepithelial cells) and are rod-shaped and flagellated, unlike coccoid-shaped and unflagellated bacteria within IBCs. Bacterial fluxing from IBCs is therefore not an essential requirement for the establishment of persistent bacterial populations in the bladder wall. The dynamic responses of IBCs to host stresses and antibiotic therapy are difficult to assess in situ. In another study, we focussed on examining the lifecycle and persistence of IBCs during early stages of UTIs. We developed a human bladder-chip model wherein superficial umbrella cells and bladder microvascular endothelial cells are co-cultured under flow in urine and nutritive media respectively. Bladder filling and voiding was mimicked mechanically by application and release of linear strain. Using time-lapse microscopy, we show that rapid recruitment of neutrophils from the vascular channel
